Chronic Care Management Program

Our Chronic Care Management Program is a vital asset for providing quality care. It focuses on patients who have two or more chronic complex medical conditions such as diabetes, COPD, atrial fibrillation, congestive heart failure, etc. The goal of this program is to help patients maintain their quality of life through medication management and a comprehensive care plan that is customized for them. An assessment of the patient’s medical, physical, mental, cognitive and psychosocial needs determines the best treatment plan for the patient.

Provider Visits

How Does It Work

The program is simple, it works just like a medical clinic, only we come to you. With the Chronic Care Management Program, the provider usually sees patients once a month. If additional visits are necessary, those can be arranged by calling Independence Provider Service. Those visits can usually occur within days versus weeks that can be the case sometimes with other services.

We accept most major health insurance plans, plus Medicare and Medicaid.

Visits are billed just the same as a regular visit to a health care provider’s office.
